Initially when I load a track the wave forms seem fine, but after the track plays for a few seconds (or when the vocals come in). The entire wave form changes to flat as seen in the attached image. I tried zooming in and out on the wave form, but that did not help any. Any one know how to resolve this bug or issue?

Is your gain knob centered?

No not all the time....is that whats causing it?

The gain will increase the size of the waveforms, so if it's too high it will indeed cause the waveforms to look like that.
